How to Use Us

Receptionist

On these pages you will find information on how to use the services we offer, general information on the surgery, how to contact us and how to find us.

In order to use the surgery, you must first be registered with us. To register with the surgery, you will need to be living within the surgery catchment area. If you are unsure whether you are in the catchment area, please call reception.

If you have a medical card please hand this into reception together with details of your home and work telephone numbers. You will be asked to complete a medical information questionnaire.

If you cannot find your medical card please advise reception and they will assist you in completing the registration form and they will also ask you to complete a medical information questionnaire.

We are ask patients for proof of identity (such as passport, driver's licence, birth certificate) AND proof of residency (such as utility bill, mortgage letter, tenancy agreement, student record).
