New Patients

You are very welcome to join our Practice.  If you are currently registered within the Chichester area you will be asked to make an appointment with one of our Doctors to discuss the reason/s why you wish to change your Surgery. When you register you will be asked to either supply your medical card or complete a registration form along with a new patient health questionnaire. You will also need to arrange an appointment with our Health Care Assistant who will carry out a short medical; this will need to be completed within a month of your registration.  All new patients are requested to bring proof of identity (passport, birth certificate, driving licence, student union card) and residency (utility bill, mortgage letter, tenancy agreement, student record).
