Diabetic Practice Nurse

Due to retirement, we are looking for an experienced Diabetic Practice Nurse to join our nursing team in our friendly, city-centre GP surgery for 23-30 hours per week. We are a forward-thinking, innovative GP Practice (training and dispensing) with a friendly and inclusive philosophy - 13,800 patients, 10 GPs with a nursing team of 7. We also have a small branch surgery in Bosham.

Duties will include:

  • Lead and deliver structured diabetes reviews, both face to face and over the phone
  • Optimise glycaemic control and cardiovascular risk management
  • Initiate and titrate diabetes medications (including injectables where appropriate)
  • Deliver patient education and promote self-management
  • Work proactively to maintain QOF performance
  • Liaise with the wider MDT internally and community diabetes teams
  • Support audit and quality improvement initiatives
  • Hours of work - 3 to 4 days per week, to include Mondays

About You:

  • Able to travel between 2 sites - Langley House in Chichester and our Bosham branch surgery
  • Is a Registered Nurse with valid NMC registration
  • Has experience in primary care diabetes management
  • Holds a recognised diabetes qualification (or is working towards one)
  • Is confident in insulin initiation and titration (desirable)
  • Is organised, data-aware, and proactive in managing QOF indicators and KPI's for a Diabetic Local Enhanced Service
  • Works well independently and as part of a collaborative team with Diabetic Lead GP, visiting Diabetic Specialist Nurse, HCA's and other members of the clinical team
  • Be organised, flexible and committed to delivering high-quality, patient centred care
  • A team player with excellent communication and interpersonal skills
