New Patient Registration
You are very welcome to join our Practice.
When you register you will be asked to complete a registration form GMS1 family doctor services registration along with a Child New Patient Questionnaire (under 16) or Adult New Patient Questionnaire (16 or over).
If you are taking regular medication you will need to see one of our GPs for your initial assessment. All new patients are requested to bring proof of identity (passport, birth certificate, driving licence, student union card) and residency (utility bill, mortgage letter, tenancy agreement, student record).
Once you are on our system you will have access to your records online via the NHS App or you can register for our online services.

Change of Personal Details
It is really important that you notify us if you change your status, address or telephone number, so that our records remain accurate. We may need to contact you at short notice and often find that our records are not up to date, particularly telephone numbers. Please view our catchment area map to see if you are still within our area as you will be asked to register nearer your home if not.

Temporary Services
If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.
To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
Named Accountable GP
All patients registered at Langley House have a GP who has overall concern for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the practice according to your choice. Indeed it may be unavoidable as your named GP may not always be available when you need them, due to their working timetable. Please contact reception if you are unsure as to who your GP is. Should you have a preference for a specific GP, we will make all reasonable efforts to accommodate this request.
